Social Worker - Adult Social Care
We have an exciting opportunity for three Adult Social Workers to join our North East Locality Team.

About the role
The postholders will be required to hold a caseload which may include the following:
- Annual reviews and reassessments of service users in receipt of care and support
- Maintaining effective case recording on Mosaic
- Participate in the duty rota
- Safeguarding
- Court Reports and Attendance
- Ensuring timely intervention for discharges following the Discharge to Assess model
- Build relationships working effectively in partnership with health colleagues, providers and other social care teams
- Participating in a weekly CSPAM panel as required
- Ensure that maximum use is made of health funding and income from client contributions for care and support is maximised
- Ensure that assessments are strength based and care and support is proportional and cost effective
- Direct payments are promoted
About You
As experienced social workers, you will manage a varied caseload, carrying out assessments focusing on the strengths of the person, their families, carers and local communities to promote independence.
You will be committed to working in a strength based, outcome focused way and to making a difference to the lives of the individuals and families which you work with.
You will probably work on cases that will change your assumptions, change your outlook and even change the way that you practise. Working in this environment will build relationships that stay with you through your social work career.
Excellent partnership working is essential to this role.
Candidates must have post qualification experience along with a good knowledge of relevant legislation and local authority responsibilities in the safeguarding of vulnerable adults.
