PFA Phase Transfer and Destinations Officer

Tower Hamlet’s vision is to enable young people with Special Educational Needs and Disabilities (SEND) to live, learn and work within their local community, achieve sustained progression, resulting in better life outcomes. We are looking for committed and proactive Phase Transfer and Destinations Officer to join our SEN service. In this role, you will be responsible for managing and tracking the movement of young people with Education, Health and Care Plans (EHCPs) as they transfer between key phases of education – secondary to post 16, and post 16 to post 19 – as well as supporting them into their chosen destinations.

 As a PfA Phase Transfer and Destinations Officer, you will liaise closely with parents, carers, schools, colleges, caseworkers and professionals to ensure timely decision making, accurate placement information and a seamless handover to the next setting. You will maintain accurate records, monitor progress against statutory timescales, and work with the PfA officers to ensure that EHCPs are updated to reflect each new stage. You will also contribute to data reporting and performance monitoring, helping the service to meet statutory duties and deliver excellent outcomes for children and young people.

 To succeed in this role, you will need excellent organisational skills, strong attention to detail, and the ability to manage multiple priorities in a fast-paced environment. You will be an effective communicator, able to work collaboratively with families and professionals, and confident in explaining processes clearly. Knowledge of the SEND Code of Practice, particularly around phase transfers, is essential. The ability to use Synergy is desirable.
